Please be advised that children are NOT to be sent to school with any medications or action plans. 

These MUST be provided to the Office to be accounted for and monitored. 

It is crucial you communicate with us regarding any changes to your child’s medical condition and/or medications. 

Additionally, I will be communicating with parents via phone or email to advise of upcoming expiries of medications and action plans. 

We kindly appreciate your ongoing cooperation!

I would like to advise all parents/guardians that if your child is unwell or has a known medical condition/illness, you may be required to provide a medical certificate from a Medical Practitioner (i.e. General Practitioner/Nurse Practitioner) to advise that your child is fit to return to school. 

As a Nurse, I have implemented this on behalf of Tarneit Rise Primary School as a requirement when deemed clinically necessary, to ensure our students remain safe and healthy to return to the school environment. 

This is also in place to get a second medical opinion from a qualified healthcare professional, further supporting your child’s health and wellbeing. 

We wish to ensure all children remain free from illnesses and diseases that may be bacterial, viral, and infectious. 

We sincerely appreciate your understanding and cooperation in this matter.
