New medical forms

Parents of children with a health condition such as Asthma, Allergy, or any ongoing medical condition will be receiving an envelope from the Health Centre in the coming weeks. 

This will contain new medical forms for 2026 to complete with your doctor so we can safely manage your child’s condition while at school, including camps and excursions. 

Please return these completed forms along with the Asthma health support plan or Allergic reactions management plan before the end of the current school year (preferably) or at the beginning of Term 1 2026. 

If you no longer think the health condition is applicable for your child, I require a signed letter from yourself and your Doctor stating this, and the medical condition will be taken off the school records.
