Secondary School Immunisation Program - Consent Forms for Year 7 & 10
The Secondary School Immunisation Program offers free schedule vaccines (National Immunisation Program) to all eligible Year 7 and Year 10 students.
Year 7 students will be offered: - Diphtheria-tetanus-pertussis (whooping cough) - Human papillomavirus (HPV)
Year 10 students will be offered: - Meningococcal ACWY
Loddon Shire Council Immunisation along with Boort District Health will be attending Wedderburn College on Wednesday 22 April 2026 to administer vaccinations to Year 7 and Year 10 students who send back a signed consent form indicating they wish to have their vaccinations.
Immunisation consent forms were sent home with year 7 and 10 students last week. If you consent for your student to have the immunisations at school, please tick and sign the ‘YES’ boxes on the consent form.
Please submit the consent form even if you do not want your student to be vaccinated or they will receive their immunisations elsewhere, indicating this by signing and ticking the ‘NO’ boxes on the form.
Please complete and return the form to the school by Friday the 27th of March.
