Year 7 Immunisation

Year 7 immunisation program, please see attached the School Immunisation Program information pack. Families can either complete a hardcopy Consent card (that is available from the RSC office) or an online digital consent.

The link below can be accessed by Year 7 families for the Online Digital Consent form for student immunisations.

When accessing the link, parents only need to enter a mobile phone number, they will then receive a text with a code to enter on the online form.

It is important for families to reply with either card or digital consent even if they Do Not Consent, this will assist with our reporting to the Department of Health.
