Year 10 Immunisations

Dear Parent/Guardian, 

On Tuesday 31st March 2026, Barwon Health immunisation service, will visit Year 10 students to administer free Meningococcal ACWY vaccine=1 injection. 

Please complete the online consent form via this URL link https://geelong.book.vitavo.com.au/school/OberonHighSchool-cDB1m4 or via the QR code.

TIPS for completing online consent

Returning Vitavo users= log into your account, tick forgot password if you are having trouble. Add your child as a new dependant( don’t edit the profile of a previous child’s record) and provide YES or NO consent for this child.

First time Vitavo users= Make your profile as parent first- Tick the button at the top of your profile that says. “Adult providing consent for dependent only” to YES

Once you have made your profile click add dependent to provide YES or NO consent for your child.
