Student Health Plan for 2026 

The Department of Education has changed the format and content details for Student health plans. A Student Health Support plan outlines how a school will support your child's health care needs,  based on advice from the student's medical team/health practitioner.  (These are separate from Anaphylaxis or Asthma Plans )

These forms can be found on the school website (forms and notices ) or on the handy links page at the back of the newsletter

Any information on these plans remains confidential and private, and is only used in a manner that will support your child.

If your child requires medication to support this condition during school hours, a medical authority form also needs to be completed by the parent. 

( We ask that the child not have the first dose of any medication at school in case of a reaction )
