Anaphylaxis, Allergy and Asthma Action Plans

A reminder to parents/carers, to please provide the Front Office with the following if your child has allergies or asthma, or is at risk of anaphylaxis:

  • An up-to-date action plan, completed and signed by a medical professional
  • Any required medication, clearly labelled with the student’s name. Medication must be in its original packaging and not expired.

     

Medications at School

As part of Lyndale Secondary College’s duty of care, prescribed medications may be stored and administered in the First Aid Room whilst your child is at school.

 

All prescribed medications must be clearly labelled with

 

  • Child’s name and Date of Birth
  • Year Level
  • Name of Medication
  • Instruction of dosage, including times to be administered and duration of course.

 

All details will be entered into the School Medication Register and students will access their medication in the First Aid Room.

 

Please be aware that sometimes circumstances beyond our control may interfere with the correct time of administration of your child’s medication. In these situations, we will endeavour to administer the medication as soon as is practicable, unless otherwise directed.

 

To assist us please complete a Medications form (can be attained from the school office) and return it, together with your child’s medication. A new form will need to be filled out on each occasion your child requires medication.

Immunisations

 

The Dandenong Immunisation Team will be visiting Lyndale Secondary College on the following dates this year to provide immunisations to students in Year 7 and Year 10. 

 

  • Year 10 Immunisations: Thursday the 16th of July 2026
  • Year 7 Immunisations: Thursday the 27th of August 2026

 

Information about how to provide consent for Year 7 and Year 10 immunisations has been sent to parents/carers via Compass. 

 

Consent forms must be completed by the 7th of March 2026.

 

  • Please complete the form, regardless of a yes or no response.
  • Completed online consent forms will automatically be returned to Councils immunisation service

First Aid - Medical Forms

Student Action Plans, Health Support Plans and Medication 

As we begin the school year, we ask that families provide the school with current medical action plans and any required medication for their child. This includes (but is not limited to) plans for asthma, allergies/anaphylaxis, diabetes, epilepsy, or any other medical condition that requires monitoring or administration while at school. Students with complex medical needs must complete a Student Health Support Plan to provide for their specific health care needs.  

 

If you are new to our school community, please forward your child’s current action plan, health support plan (if required), medication and medication authority form as soon as possible so we can ensure your child’s wellbeing and safety while in our care. 

  • Please ensure action plans are signed by a medical practitioner and are in date 
  • Medication must be clearly labelled in original packaging with your child’s name and dosage instructions 
  • A Medication Authority Form must be completed by a parent/carer 
  • Medication must be delivered to the school office by a parent/carer

 

If your child’s medical needs change at any time throughout the year, please notify us so we can update our records. Thank you for your support to help us maintain a safe environment for all students.
