Dear Parent/Guardian,
On Tuesday 16th September 2025, Barwon Health immunisation service, will visit our Year 7 students to administer the free
- HPV( Human Papillomavirus) vaccine =1 injection
- Boostrix( Diphtheria, tetanus, and whooping cough booster) = 1 injection
- Make your profile as parent first- Tick the button at the top of your profile that says. “Adult providing consent for dependent only” to YES
- Once you have made your profile click add dependent to provide YES or NO consent for your child.
- You will receive a confirmation email once consent is completed.
