At Nazareth, we are committed to ensuring every student receives the care and support they need to thrive. Our school community is wonderfully diverse, and many of our students live with a wide range of medical conditions. We want to make sure that the right supports are in place and that we are not only compliant with regulations but genuinely responsive to your child’s needs. 

 

If your child has a diagnosed medical condition and may require support while at school, we encourage you to reach out to the Health Centre as soon as possible.

To ensure your child’s safety and appropriate care, schools are required to have current medical documentation on file.  This includes: 

  • A Condition-Specific Action Plan (e.g. Asthma, Anaphylaxis, Allergy, Diabetes, or Epilepsy), signed by a Medical Practitioner, and 
  • An Individual Management Plan (IMP), signed by a Parent/Guardian/Carer

Both documents must be reviewed and updated annually

 

Individual Management Plans helps us to ensure clear communication between the College, the student and their parent/guardian/carer, so we can provide the right supports and respond appropriately in both everyday situations and emergency treatment that your student may require. 

  • When students are taking prescribed medication (for conditions such as ADHD) and are required to take this either at school, or if participating in an excursion or overnight camp we also require documentation via a Medical Management Plan and Medication Authority Form (Doctor Signed).  

We understand that managing medical documentation can be overwhelming, and we’re here to help guide you through the process. We have sent home this week via Heads of House, outstanding documentation requests that we require. These plans ensure we understand the supports your child requires and outline clear steps for managing emergencies. We appreciate your assistance in getting this documentation completed.  

 

Students with Asthma, Anaphylaxis, Allergies, Diabetes, or Epilepsy can use the condition-specific action plan (signed by a Medical Practitioner) in conjunction with an Individual Management Plan (IMP - parent/ guardian/ carer signed). All documentation requires annual review. The Medical Management Plans are designed to identify supports required and procedures for emergency management of medical conditions.
