General Information

Medications At School
All medications required to be taken by students at school MUST
be delivered to Admin staff with the following:
- Medical Authority Form - fully completed & signed by parent and
- Medication in original packaging with student name/dosage/prescribing doctor
Ongoing medications must also have a letter attached from the medical professional detailing child/medication/dosage OR the Medical Authority Form is signed by the medical professional to approve the form details.
Medical Authority Forms can be obtained at the office or by email as requested.
Allergies
At St Kilian’s School we have a number of children with allergies
that can be life threatening.
Please exclude nuts when packing your child’s lunch.
