School Dental Van

Dental Van Visit

The Australian Smile Group Dental Van will be visiting St Joseph’s Primary School for all our Prep to Year 6 students across the school in Term Four on Tuesday 1st December until Friday 4th December. 

If you would like your child to use this service please read and complete the form that has been sent home. Forms will need to be returned to the school office or to your student’s classroom teacher by the end of this term. Please note that we are required to send the forms off on the final day of this term so no later forms will be accepted. 

All staff from the Australian Smile Group hold a working with children check and there will be a member of our St Joseph's staff present during the visits. 

You consent is required and the sercice may include performing some minor dental procedures on this day. If you do not consent to this we ask that you do not give permission.
