Medical Documentation

As first Term draws to a close, I would like to thank all the Parents who have returned all the Medical documentation required to support your children both at school and whilst they are on excursion. 

We need current Emergency plans for Asthma, Epilepsy, Midazolam, Allergies and also Medication Authorities to ensure we are giving your child the correct support required at school. These plans need to be signed by a doctor to ensure a current medical endorsement.

For those who are yet to return your documentation please forward it to the school as soon as possible as excursions are already being planned and current paperwork is required.

Please contact the school if you need any new blank copies of the required paperwork.

Immunisations

A Student immunisation day is scheduled for Wednesday 21st of May at 1.30pm and consent forms will be sent home this week for both year 7 and 10 vaccinations and Influenza.  

Please complete and return forms Yes or No

Forms are to be returned to school by Monday 28th of April for the council to collect.  

Booroondara Immunisation Service has advised that parents who choose to take part in school vaccinations the Council Immunisation Nurses will be contacting you directly regarding their child’s vaccination experience.
