A note from the Sick Bay

(A message from the School Nurse)

Welcome back to all families at EVPS!  We continue to organise all our student’s health care needs for the year. At the end of 2025 your child, if they’re known to have a health alert, was sent home relevant medical/action plans to be completed by their medical practitioner. This is a timely reminder to return them completed to the office as soon as possible.

 

  • If your child has asthma, even if its seasonal, an asthma action plan must be provided. It needs to be completed and signed by your doctor. A spacer and Ventolin must also be supplied to school. These will be placed into a kit clearly labelled with your child’s name and kept within the sick bay, ready for use as your child needs it. 

 

  • If your child suffers from anaphylaxis (RED ASCIA PLAN) or an allergy (Green ASCIA PLAN) ALL forms must be completed and signed by your doctor. Please also supply the medication listed on the plan clearly labelled in original packaging, with your child’s name. 

 

  • If your child requires medication to be given at school, this must be provided to the main office/sick bay, with a signed medication authority form completed by the parent. Medication must be clearly labelled with your child’s name, grade, be in original packing and prescription as required. 

 

  • PLEASE NOTE: Where possible- medications should be administered outside school hours. This includes ANTIHISTAMINES - if your child suffers from hay fever like symptoms, providing a dose before school is optimal and deters disruptions to their learning day.

 

Please return all completed forms to the Admin Office at the beginning of the 2026 school year.
