Medical Information – PAM Medical

Parents and carers are reminded of the importance of ensuring their child’s medical and health information remains current and accurate throughout their time at the College.
Up-to-date medical information allows the College to appropriately support students, respond safely in the event of illness or a medical emergency, and ensure any necessary supports or reasonable adjustments are in place.
Keeping Your Child’s Medical Information Up to Date
Parents/carers are asked to regularly review their child’s information in PAM Medical and advise the College whenever there is a change to their child’s:
diagnosed medical or health condition
medication or treatment
medical management requirements
mobility or personal care needs
treating medical or health practitioner advice
emergency management requirements.
Significant or temporary changes that may affect a student’s participation at school should also be communicated to the relevant Year Level Leader and/or College Nurse.
This may include, for example, a student temporarily requiring crutches, a moon boot or another mobility aid; returning to school following surgery, injury or hospitalisation; or requiring temporary reasonable adjustments to safely access classes and College activities.
Medical Management Plans and Student Health Support Plans
Students with diagnosed medical or healthcare needs may require a current Medical Management Plan or condition-specific management plan completed by the appropriate registered medical/health practitioner.
This may include conditions such as anaphylaxis, asthma, diabetes, epilepsy and other diagnosed medical or healthcare conditions.
Where required, the College will also develop a Student Health Support Plan (SHSP) in consultation with parents/carers and with reference to the medical advice provided by the student’s treating practitioner.
These documents provide important information to College staff about the student’s condition, day-to-day support needs, management strategies and the appropriate response should symptoms occur or the student require medical assistance.
Reviewing Medical Plans
Medical Management Plans, condition-specific plans and Student Health Support Plans must remain current and should be reviewed at least annually, or at the review interval specified on the relevant plan.
Plans must also be reviewed and updated whenever there is a significant change to the student’s condition, treatment, medication or support needs, or where updated advice is provided by the student’s treating medical/health practitioner.
Parents/carers are responsible for providing the College with updated medical documentation when required. While the College may issue reminders when documentation is approaching review or expiry, it remains the responsibility of parents/carers to ensure the College has current and accurate medical information for their child.
Medication at the College
Where medication is required during school hours or College activities, including camps and excursions, the appropriate Medication Authorisation Form must be completed and provided to the College.
Medication supplied to the College must be provided in accordance with College medication procedures, including being in date, appropriately labelled and supplied in its original packaging.
For students at risk of anaphylaxis, parents/carers must ensure the College is provided with the student’s current ASCIA Action Plan for Anaphylaxis and their prescribed adrenaline injector.
Working together to support your child
Effective medical management relies on ongoing communication between families, students, treating health practitioners and the College.
Please do not wait for an annual review or reminder if your child’s health circumstances change. Parents/carers are encouraged to update PAM Medical and contact the relevant Year Level Leader and/or College Nurse as soon as practicable so that appropriate supports can be reviewed and implemented.
Keeping this information current helps ensure College staff have the information they need to support your child safely and appropriately at school, on excursions, at camps and during other College activities.
