Student Medical Plan

This a reminder that any student with an identified health care need, identified personal care need, and/or dietary preferences/restrictions need to complete a Student Health Care Plan. If you have already completed one on enrolment, please update any changes that may have occurred since the original plan was submitted. New forms are available at the office. 

Students with diagnosed Anaphylaxis, Asthma, and/or Allergies, only require an ASCIA Action Plan relevant to their condition. Please ensure that plans and medications are up to date. They do not require a Medication Authority Form, provided the Action Plan authorizes any prescribed medication and dosages. 

 

If the student takes medication during school hours for their condition, the Medication Authority Form must be completed by a Medical/Health Practitioner or parent/carer. 

All medications to be taken during school hours/excursions/camps must be brought to school in their original package, with the pharmacy label attached (if applicable), and be within the expiry date. Please drop into the office.
