Medical Management Plan

If your child has been diagnosed with a specific medical condition or has complex medical needs it is vital that you provide relevant information to the school so that we can ensure your child receives the appropriate care necessary to manage their condition and keep them safe and well while at school.

 

In order to support students with a medical condition it is a DEECD requirement that parents/carers provide the school with a current personal Medical Management Plan prepared, signed and dated by your child’s doctor/specialist. This management plan needs to be updated every year and in line with any changes.

 

If your child has a significant medical condition and you have not provided the school with a current medical management plan in the last 12 months please make arrangements to see your child’s doctor to have the appropriate forms completed and returned to school.

 

Any medications that your child needs to have administered at school should be documented on a Medication Authority Form with doctor’s instructions, also signed and dated by your child’s doctor/specialist or pharmacist.

Sunsmart Reminder

Over exposure to the sun presents a serious health risk. Australia has the highest rate of skin cancer in the world. It is estimated that more than 75% of skin cancers could be prevented by practising sun protection in early childhood and in adolescence.  Students must be educated as to the need for suitable sun protection and must be protected from over exposure to the sun whilst at school.

 

The Concord School Sun Smart Policy states:

·         Primary students are to wear hats whenever they are outside from October to           April (terms 1 and 4) as this is when UV Index levels reach 3 and above.                   Primary students without hats will be restricted to playing in designated                       shaded areas.

·         Secondary students are strongly encouraged to wear hats whenever they are           outside from October to April (terms 1 and 4). Secondary students without                 hats will be encouraged to remain in shaded areas.

·         Students will be actively encouraged to wear a broad-spectrum water resistant           sunscreen (SPF30+). Parents/carers are encouraged to apply the sunscreen             at home.  Sunscreen can only be applied at school by students, under the                 supervision of a teacher, according to the manufacturer’s directions.

·         The school uniform shop will have broad-brimmed hats and sun-protective                  clothing, such as long sleeve polo shirts, available for sale.

 

Please ensure that throughout term 4 your child has:

·         Their sun hat available at school at all times

·         Had sunscreen applied prior to arrival at school

·         Their own sunscreen sent to school for re-application on days that they are               outdoors for prolonged periods

 

It is also vital to our health to remain hydrated especially during the warmer months.

 

Please provide your child with a full water bottle daily.
