IMPORTANT MEDICAL INFORMATION

Students with Asthma and Anaphylaxis

Please make sure that we have your child’s most recent Asthma and/or Anaphylaxis Action Plan.

 

The plan that we are referring to is the one that has the patient’s photo attached and is signed off by a doctor in the current year (2021).

 

According to ASCIA, Australia’s authority on anaphylaxis, the plans should be renewed by your doctor at least once a year.

Immunisations - (RETURN FORMS EVEN IF NOT RECEIVING VACCINE)

The Kingborough Council’s immunisation team will be providing vaccinations to year 7 and year 10 students this year.

 

The following vaccines will be provided to year 7:

Human papillomavirus (HPV) – two doses given 6 months apart

Diphtheria, tetanus and pertussis (whooping cough) dTpa – one dose

 

The following vaccines will be provided to year 10:

Meningococcal ACWY – one dose

Please complete and return all the consent forms IMMEDIATELY (even if your child will not be receiving the vaccines). Forms were due today, Wednesday 10 February 2021.

 

If you do not return your child's form you will need to contact the Kingborough Council directly or your family doctor to get their immunisations up to date.

Accessing Immunisation History Records

The fastest way to access your child’s Immunisation History Statement is online through your Medicare online account through myGov 

or using the Express Plus Medicare mobile phone app.  To use the app, you will need to set up your Medicare online account through myGov.

 

Alternatively, you can:

  • ask your vaccination provider to print a copy for you from the AIR site
  • call the AIR on 1800 653 809 (8am-5pm, Monday to Friday) and request a copy to be mailed to you (this can take up to 10 business days)
  • request a copy at a Department of Human Services Service Centre.

You can only access your child’s Immunisation History Statement if they are under 14 years of age.  If they are 14 years of age or over, your child must access their own Immunisation History Statement or give DHS permission to provide you with that information.

 

You can save and email the statement directly to your child’s pre-school or child care centre.

  • a fever
  • runny nose
  • cough
  • sore/itchy throat, or
  • shortness of breath.

If you have serious symptoms, such as difficulty breathing, call Triple Zero (000) and ask for an ambulance.
