Immunisations Year 7 & 10

Year 7 and 10 students were given an Immunisation form to bring home

If you student did not receive a form one can be collected from the office.

Could we ask that the Year 7 forms be completed and returned to the box in the office by Friday 26th March. These forms must be returned whether your child is having or not having the vaccination.  Immunisation date is 11th March at the College.

We request the Year 10 forms be returned by Thursday 4th March, to the box at the College office. These forms must be returned whether your child is having or not having the vaccination. Immunisation date is 22nd April.
