Head Injury/Facial Injury Alert

Should your child be referred to a Medical Practioner or Emergency Department during school, after school, on the weekend or during school holidays following a Head injury, Neck or Facial Injury and your child is diagnosed with Concussion, it is imperative the Parent/Guardian submit a Medical Clearance Certificate to the College nurse stating when your child can return to sport or other activities at the College.

Please inform the College Nurse as soon as possible if there are any changes to your child’s medical information.
