Student Medical Plans

When enrolling your child at Albanvale Primary School, parents informed us on the enrolment form of ongoing medical conditions. For the health and wellbeing of all students, a yearly update of Medical Plans needs to be completed by doctors and parents/guardians.

These include Asthma, Anaphylaxis, and Allergy plans. These need to be filled out by your child’s Doctor and returned to school. 

If your child takes ongoing medication, we also require an updated Mediation Form to be filled out.

Over the next few days, we will be sending home new Action Plans and Medication Forms to be completed and returned to school for those out of date and require urgent attention. If you do not receive one, it means the school has updated plans. 

Please note, we are not able to administer medication without the appropriate authority forms.
