Updating Medical and Contact information

Please inform the College Nurse if there are any changes to your child’s medical information.

 

Please inform College Administration office if there are any changes to your contact/emergency information.

 

 

College Medication procedure

If a parent requests the College Nurse administer medication to their child, they must complete a Medication Request Form, this is available from the Health Centre. The medication must be in its original container or packaging and clearly labelled with the following:   

  • Child’s name
  • Name of medication              
  • Expiry date
  • Dose and time to be administered
  • If a student is required to carry and self-administer medication while at school, only a single measured dose for one school day can brought onto the College premises. Medication must be locked in that student’s locker.
  • Students with diagnosed Asthma are requested to carry their reliever medication with them at all times. Students must have their Asthma medication with them during off-site and out of school activities eg, sport, camps etc.
  • Students with diagnosed Anaphylaxis are requested to carry their EpiPen (adrenaline auto-injector) with them at all times (age appropriate). Students must carry their EpiPen to all off-site and out of school hours activities eg, sport, camps etc.
  • Students with diagnosed Type 1 Diabetes are requested to carry their “Hypo Kit” at all times.

Students must have a “hypo kit” with them during off-site and out of school activities eg, sport, camps etc. 

Year 7 Immunisation Program

The Department of Health is offering parents of students in Year 7 the opportunity to have their child vaccinated against:

  • diphtheria, tetanus, pertussis (whooping cough)- 1 injection.
  • human papillomavirus (HPV)- 2 injections at least 6 months apart. It is important your child receives both doses for best protection.

The Department of Health vaccination team will be visiting the College on Wednesday 23 March to administer the first round of vaccines. Information packages and consent forms will be sent home in due course. 

 

Please note that all students are vaccinated in school by a Nursing team with expertise in immunisation.
