Year 7 Vaccination

The Department of Health is offering parents of students in Year 7 the opportunity to have their child vaccinated against:

  • diphtheria, tetanus, pertussis (whooping cough) - 1 injection.
  • human papillomavirus (HPV) - 2 injections at least 6 months apart. It is important your child receives both doses for best protection.

The on-line VaccinateWA system is now to be used by parents/carers to complete the vaccination consent form in order to consent to their children/dependents receiving vaccinations whilst at school.  

 

A paper-based immunisation consent form can also be obtained via the HealthyWA  website or from the College Health Centre (limited amount available) This must be returned to the College even if consent is declined.   On-line consent process is preferred.

 

To provide consent for vaccination, please register your child onto VaccinateWA.

Please register consent by 31 March, 2023.

 

The Department of Health Vaccine Team will be visiting the College on Wednesday 26 April to administer the first round of vaccines.
