Reminders from Sick Bay:
Year 7 Camp:
Please ensure that your child’s medical forms are up to date. If your child needs to take medication whilst on camp, they will require a ‘Medication Authority Form’. Any medication to be sent to camp must be in its original packing, clearly labelled with students name, form, ID code, expiry date and dose instructions.
If you require new medical forms, please come see Tam or Corinna in sickbay, alternatively we can also send a digital copy via email.
Unfortunately, if forms aren’t returned then your child may not be allowed to attend camp.
Immunisation:
Please return Immunisation consent forms back to sickbay prior to 14/2/2023 so we can send information to the City of Whitehorse.
All Year Levels:
Please ensure that your child’s medical forms are up to date. This information is updated annually. If they’re are any changes to your child medical condition or treatment updated information will be required by the college.
You can also assist us by ensuring you keep us up to date with any changes throughout the year.
If you require new forms please send your child to collect or contact the school (via phone or email) so we can send out digital copies out via email.
