Reminder to families of students with medical conditions

Please ensure medical plans are supplied to school

If you have a child with any of the following medical conditions, a Medical plan that has been completed by a medial practitioner, (i.e. Doctor) must be supplied to the school.

• Anaphylaxis
• Asthma
• Allergy *
• Diabetes *
• Epilepsy *

(*medication authority form must also be filled if supplying medication to the school)

The Department of Education and Training have strict guidelines regarding the type of medical plans schools are allowed to accept, the medical plan templates can be collected from the General Office.

If you HAVE NOT already supplied a medical plan for your child and your child has any of the above conditions a plan MUST be supplied to the general office no later than Thursday 26 March 2020.
