Request for Action Plans

Dear Parents and Carers,

 

Welcome to Term 2.

 

This is a courtesy reminder to please ensure that, if your child has a diagnosed medical condition, you have provided the College with the necessary paperwork and medication.

 

Anaphylaxis:

Please provide a copy of your student’s ASCIA Action Plan for Anaphylaxis (prepared by a medical or nurse practitioner). Please attach a recent photo of your student. The plan must outline the student’s known severe allergies and the emergency procedures to be taken in the event of an allergic reaction. This needs to be reviewed by your student’s medical practitioner every one to two years. You can find a template for this below:

 

Please also ensure that you have provided the College with an EpiPen that is within its expiry date. This will be kept in the General Office in a container clearly marked with your student’s name and photograph. 

 

Allergies:

If your student has been diagnosed with a mild to moderate allergy, please provide a copy of your student’s ASCIA Action Plan for Allergic Reactions (Green Plan). This should be prepared by your student’s medical practitioner and reviewed every one to two years.

Note: If your student has been diagnosed as being severely allergic (anaphylactic) to a particular allergen but also has other diagnosed mild to moderate allergies, please only use the Anaphylaxis Action Plan, and ensure that all allergies are listed on the one plan. A template can be found below.

 

Please ensure that you provide the College with any medication listed on the Action Plan. This will be kept in the General Office in a container clearly marked with your student’s name and photograph.

 

Please also complete the following Medication Authority Form.

 

 

Asthma:

If your student has diagnosed asthma please ensure that an Asthma Action Plan, completed by your student’s medical practitioner, has been provided to the school. The plan must outline your student’s known asthma triggers and the emergency procedures to be taken in the event of an asthma flare-up or attack. This should be reviewed by your student’s GP annually.

 

The Asthma Action Plan should contain:

  • the prescribed medication taken and when it is to be administered (for example, on a regular basis, as premedication to exercise or if your student is experiencing symptoms)
  • emergency contact details
  • contact details of your student’s medical or health practitioner
  • details about deteriorating asthma, including signs to recognise worsening symptoms, and what to do during an attack or what medication is to be used.

Diabetes and Epilepsy:

Please provide a condition-specific health management plan to the College. This can be provided by your student’s GP.
