Year 10 Meningococcal ACWY Vaccination

The Department of Health is offering parents of students in Year 10 the opportunity to have their child vaccinated against Meningococcal ACWY.

Parents can also register consent for the COVID-19 vaccine for their child.

The online VaccinateWA system is now to be used by parents/carers to complete the vaccination consent form in order to consent to their children/dependents receiving vaccinations whilst at school. 

To provide consent/or decline vaccination, please register your child onto VaccinateWA.

A paper-based immunisation consent form can also be obtained via the HealthyWA  website or you can complete the consent form attached and return this to the Health Centre by Friday 2 June even if consent is declined.  

Online consent process is preferred.
