Year 10 Meningococcal ACWY Vaccination

The Department of Health is offering parents of students in Year 10 the opportunity to have their child vaccinated against Meningococcal ACWY. Parents can also register consent for COVID 19 vaccine for their child.

The on-line VaccinateWA system is now to be used by parents/carers to complete the vaccination consent form in order to consent to their children/dependents receiving vaccinations whilst at school. 

To provide consent/or decline for vaccination, please register your child onto VaccinateWA.

A paper-based immunisation consent form can also be obtained via the HealthyWA  website or you can complete consent form attached and return this to the Health Centre by Friday 12 May even if consent is declined.  Online consent process is preferred.

The Department of Health Vaccine Team will be visiting the College on Thursday 8 June to administer Meningococcal ACWY Vaccine.

Please register consent/or decline by Friday 12 May.
