Medical Information

Saint Helena's Catholic Primary School must know of all student medical conditions.

If your child has an allergy, asthma or anaphylaxis, we require an Action Plan completed by a Medical Practioner along with the School Medical Action Plan and all relevant in date medication.

For all other conditions please supply the school with a completed Medical Action Plan.
