Dear Parents,

If your child(ren) has a medical diagnosis of either, and, or Anaphylaxis/Asthma/Allergy with an accompanying Action Plan that has been signed and dated by a medical practitioner, and with a current photo, please upload this onto OPEROO and provide a hard copy to the school with any listed medications ASAP.

Generally, we require that Action Plans are reviewed every 12 months.
