Year 9 Vaccines 

Dear Year 9 Parents and Guardians, 

Year 9 Meningococcal ACWY vaccines will take place on campus on Wednesday 25th May.

Consent forms were sent in the mail at the end of last term and are attached again for your convenience. Thank you to those parents who have already returned the forms.

If you haven’t returned the form, please complete the form attached and have your student return all signed consent forms to Student Access no later than Monday 23rd May.  

Students will only be vaccinated with returned, completed and signed consent forms.
