Vaccinations for Aboriginal Students Years 9 & 10 only

Dear Parents and Guardians, 

Year 9 and Year 10 Aboriginal students Influenza and Pneumococcal vaccines will take place on campus on Wednesday 25th May.

Consent forms were sent in the mail at the end of last term and are attached again for your convenience.

Thank you to those parents who have already returned the forms.

Please have your student return all signed consent forms to Student Access no later than Monday 23rd May.  

Students will only be vaccinated with returned, completed and signed consent forms.
