2022 Medical Documentation

Dear Parents and Carers,

In preparation for meeting the healthcare needs of your child in a safe and current manner, we will be sending out new medical forms regarding health emergencies and medication to be completed and signed by your child’s health care practitioner for 2022. It is a Department of Education and Training as well as a Belmore School requirement for your child’s onsite attendance and excursion participation, that all Emergency Health Plans and Medication Authorities be updated and provided to the school annually. 

In addition this year, we will be including a comprehensive personalised Belmore School Health Care Plan which has been prepared based on our knowledge of your child. We request that you read this plan closely to ensure we are meeting all of your child’s current and ongoing health needs.  Please edit and detail any required changes for us to incorporate into the Health Care Plan to ensure we are working in close partnership to fully meet your child's health needs to support them to attend Belmore School within a safe and supported environment.
