Youth Integrated Stepped Care Project

We would like to introduce the Youth Integrated Stepped Care Project to young people, families, friends, support people and service providers.

This project sees young people, families and services providers working together to develop an innovative, comprehensive youth specific stepped care model for the eastern corridor of Eastern Melbourne PHN. This aims to enhance access to the full spectrum of mental health, AOD and suicide prevention services and supports across the continuum of stepped care, ensuring the right care and the right service at the right time, to improve outcomes for young people and their families.  The project presents a real opportunity to improve the lives of young people aged 12-25 in the eastern region of Melbourne.

Please feel free to share this letter via your networks, notice boards and newsletters
