Administration of Medication at the College

In accordance with our Medication Administration Policy, College staff are unable to administer any medication, including for example paracetamol to students without a parent returning a fully completed and signed Authorisation for the Administration of Medication at School formThis form provides us with your consent to enable our staff to administer medication in accordance with your child’s Medical Management Plan.    In addition to the form, should your son / daughter require medication, this must be provided in the original packaging with clear dosage instructions.  This medication, together with your son / daughter's form will be stored in the College Office in a sealed box, labelled clearly with your child's name and photo identification.

If you require an Authorisation for the Administration of Medication at School form, please download from the link below or contact College Office on (03) 6216 7900 and we can arrange for a copy to be emailed or sent home in hard copy with your child.

We thank you for your assistance in ensuring that your child’s medication records are in order to maintain their health, safety and wellbeing whilst at school.
