It is also a Department for Education requirement that all students with a medically diagnosed health condition have a medical plan updated annually with the school.

If your child has a medically diagnosed health condition, please provide the school with: 

Individual Care Plan signed by Medical Practitioner(GP or Specialist) or Nurse Practitioner Medication Agreement signed by the parent/caregiver and up-to-date emergency contacts. If your child requires medication to be kept onsite, please ensure that: it is within its expiry date, preferably to December 2021. It is clearly labelled with student details on a pharmacy label. If your child needs to take medication whilst at school, we will need the following documents:

1.      Medication Agreement (HSP 151)

2.      Individual Care Plan eg Asthma, Anaphylaxis etc or Non-Specific Health Care Plan (HSP 110) 

3.      For those children carrying and or self-administrating medication the form (HSP 154) will be need to be filled in by parent/caregiver
