Primary School Nursing Program

Dear Parents/Carers, 

The Primary School Nursing Program is a free service offered by the Department of Education to all prep/ foundation students and their families during their first year of school. 

Referrals for Year 1-6 students are also accepted with teacher consultation. 

All Prep parents will have received information last week regarding the online referral form and questionnaire to access the health check for their child.  A paper version of the questionnaire is available if preferred. Please see the office.

Please complete these forms by the end of the week.

Primary School Nurse, Michelle Chappell will visit the school on Thursday 20th Feb and Friday 21st Feb to complete the health checks. 

This is a great opportunity for a free vision and hearing check and follow up any other concerns regarding your child's general health and wellbeing.
