Hazard: Something could occur

Near Miss (or a Close Call): Something did occur but there was no harm caused

Incident or Accident: Something did occur and harm was caused

 

Everyone should be more proactive in reporting all hazards and near misses in your work area, whether travelling out on the road, warehousing, manufacturing products, servicing equipment, in store, even in the office complexes.

 

Current data from recent research in this area shows that approximately 90 near misses precede one serious accident at work. However, that single serious accident can result in severe injuries, illness and even the loss of human life. So, it is vitally important to make a conscious effort to report every occasion when you have experienced a hazard, near miss or close call.

 

As you can see below Hazard Reporting has dropped off (blue) and what we do not want is the Recordables (orange) to lift off the zero line, if it does that means we have injured one of us and that “one of us” could be you or a co worker.
