Student Health and Medical Requirements

All students with a medical condition or specific health needs are required to complete the relevant forms when they commence at our school.

If your child has asthma, anaphylaxis, allergies, diabetes, or epilepsy, you must provide the school with their current ASCIA Action Plan along with the necessary medication.

Families of children with anaphylaxis are also required to complete both a Student Health Support Plan and an Individual Anaphylaxis Plan.

If your child requires medication not related to an ASCIA Action Plan—such as antibiotics, Panadol, eczema cream, or eye drops—a Medical Authority Form must be completed before staff in First Aid or the office are able to administer the medication.

Please note that all medication must be delivered by parents to the office. For safety reasons, medication cannot be placed in school bags or delivered by students.
