Medical Management Plans

I have recently sent out requests for new Allergy Plans and Asthma Plans for the new school year. 

We are also now required to have Health Support Plans and Allergy Management Plans completed for any child with these conditions; I have attached these to the new plan requested. 

Please return the completed plans in the new school year. 

It is important that we receive up to date plans so we can safely manage your child’s health condition when they are at school, and on excursions and camps.

Please complete these forms with your Doctor and we also need them signed by your Doctor. 

If you believe your child no longer has this condition, please get a signed letter from your Doctor stating this and we will take them off your child’s record.
