Dear Parents/Carers,

 

This is a friendly reminder for you all to please make sure all of your emergency contact numbers, home address and medical information/medication is all up to date for your child/children at our school.

 

If you would like to advise the school of a change in phone numbers, emergency contacts, address, medical information or any other information you would like updated, please inform the school via email at dl.1066.info@schools.sa.edu.au

If you are notifying of a change of address, the following documents are required:

  • a recent council rates notice-if you own your home
  • a rental agreement that covers the first 12 months-if you are renting
  • a recent gas or electricity bill for the property

To update medical information, an updated Medical Plan is required from your Doctor.
