FOUNDATION SCHOOL NURSING PROGRAM
Dear Prep Parents and Carers,
Welcome to you and our foundation children.
The Victorian Primary School Nursing Program (PSNP) is a free health assessment service offered by the Department of Education to all children during their first year of primary school.
The School Nursing Program continues to offer support to Foundation students and families through the School Entrant Health questionnaire (SEHQ).
An envelope will be sent home today with your child regarding the School Nursing Program (includes QR code and link for parents/carers to access the SEHQ)
This contains information regarding an online questionnaire about your child's health and development.
This questionnaire enables you to raise any concerns you may have regarding your child's health.
Please complete the online questionnaire by 19 February, 2025.
Every Prep to Year 3 student in 2025 at participating schools will be eligible to receive a free vision screening and, if required, further eye testing and free glasses.
A consent form and information brochure will be sent home today with your child.
Please complete the consent form and return it to the office/class teacher by Friday 6th December 2024.
