HEALTH CARE CARD

Do you currently hold a Health Care Card?

If you have a valid, income-tested Health Care Card and was valid from the beginning of 2025 school year, the following fee assistance is available. State Government of Victoria, Department of Education & Training, Camps, Sports & Excursions Fund (CSEF) provides payments for eligible students to attend camps, sports and excursions:

• Families holding a means-tested concession card (must be in parents name not your child’s) or temporary foster parents are eligible to apply.
• The annual CSEF amount per primary school student in 2025 is $150.
• Payments are made directly to the school and are tied to the student.
• If you hold a valid Health Care Card, please complete the attached CSEF Application Form and return with this form

IF YOU HAVEN'T ALREADY SUPPLIED A COPY OF YOUR CARD TO THE SCHOOL OFFICE PLEASE DO SO BY THE END OF MAY
