Enrolment Information 

For families who have received follow-up emails regarding the submission of pending documents, we kindly ask that you submit these as soon as possible to finalise your child's enrolment. 

 

To assist you, please find below the PDF files of the consent forms and list of supporting documentation to confirm your child's details.

 

  1. Birth Certificate 
  2. Passport and Visa Documentation (if applicable)
  3. Medicare Immunisation Certificate: How to obtain Medicare Immunisation Certificate?
  4. 100 - Point Evidence of Address 100-point Residential Address Check
  5. Medical plans, eg, Asthma Action Plan, Anaphylaxis Plan, Allergy Plan
  6. If applicable, assessment reports and court orders
