Student Information/Permission forms
I sent home student information packs the other week.
Thankyou to those families who have returned them.
Can those still to return the forms please review, make any changes necessary or mark "no changes" and return to the school office as quickly as possible.
If you have misplaced your form please let me know and I can print another for you.
Some families still need to supply Head lice inspection permission forms, Birth Certificates and/or Immunisation Certificates. You would've had a reminder in your pack. We need this information for compliance reasons.
The Miscellaneous Permission form also needs to be filled in, signed and returned.
New families will have had CSEF application forms included as well (See info below).
