InglewoodTrainee

IDHS is situated in the Shire of Loddon and provides services to more than 5,500 residents in the communities of Bridgewater, Inglewood, Korong Vale, Newbridge, Serpentine, Tarnagulla, Wedderburn and surrounding townships.

All IDHS facilities are conveniently located on one site – Urgent Care Centre, community health and hospital and residential aged care (hostel and nursing home). IDHS also offers a diverse range of services including diabetes education, community and district nursing, counselling and cardiac rehabilitation, planned activity groups, volunteer and strength training programs.

IDHS has strategic alliances with a range of hospitals, health services, health alliances and government bodies.

THE POSITION

The primary role of the Personal Care Worker (PCW) Trainee is to work as a productive member of the care team to provide person-centered care. The PCW Trainee is expected to provide a competent level of personal care, cleaning and food services for residents and to work within the limitations of their role. The PCW Trainee works under the direction and support of the supervising Registered Nurse (RN) at all times. The PCW Trainee will complete a Certificate III in Individual Support (Aged Care).

KEY RESPONSIBILITIES AND DUTIES

  • Provision of person centered care
  • Undertake assessments as directed by the RN
  • Provide information to the RN/Enrolled Nurse (EN) to help with developing care plans for residents
  • Provide personal care to residents as described in their care plans
  • Provide food and cleaning services as scheduled
  • Check on residents regularly, report changes to the RN
  • Document in simple, non-judge mental and plain language the progress of residents and any changes, events or incidents
  • Communicate relevant information about residents to the next shift in a confidential manner
  • Contribute to discharge and transfer planning by reporting findings to the RN and completing transfer forms as directed by the RN

Critical thinking and analysis

  • Contribute to quality activities/audits as directed by the RN
  • In consolation with the RN, reflect/think about things that did not go as planned on shift and what leanings can be taken from that

Working relationships and safety

  • Develop good working relationships with residents and their relatives
  • Demonstrate competent communications kills
  • Provide basic information to residents and refer to the RN for more complex needs
  • Ensure care is provided in a safe environment by discussing risks and problems with the RN
  • Work positively and pleasantly with staff from all areas of the health service
  • Keep up-to-date and follow organisational policies, procedures and guidelines

 Professional practice

  • Work ethically within the Aged Care Act requirements and standards
  • Ensure a sound understanding of the role of the PCW
  • Work within the limitations of the PCW role
  • Work under the direction and supervision of senior staff

ABOUT YOU

  • Willingness and commitment to completing a Certificate III in Individual Support (Aged Care)
  • Willingness to gain experience in aged care
  • Understanding of the concept of person-centered care
  • Good communication skills
  • Commitment to positive working relationships
  • Effective written and verbal communication skills
  • Demonstrated awareness of confidentiality and privacy principles

REQUIREMENTS

  • National Police Record Check A current and satisfactory National Police Record Check must be completed by all new staff prior to commencement
  • Immunisation & Vaccination As a health provider dedicated to providing quality patient care, we all need to be aware of the critical importance of infection control. Each staff member has a responsibility to comply with IDHS’ Staff Immunisation Policy and various infection control policy and procedures. All staff are required to provide evidence of vaccinations prior to commencement at IDHS
