Nforma Medical
This week the medical information held in Operoo was transferred to NForma and went live.
We will require parents to look at their child's medical records and check that the information transferred is accurate. This is a time to make any changes required. Having accurate records is essential for your child/children's safety and wellbeing. In an emergency, this is what we use. If there is documents or information missing, we will require parents to upload this.
Parents need to check and correct Medical information on Nforma by Wed 19 November. Please upload:
- medical permission forms if your child has to take any medication at school
- Asthma Management Plan
- Allergy / Anaphylaxis Management Plan
- Any other documents relevant to your child's medical management
NForma Forms for Excursions in USE!!!
NForma - Forms communicate on activities such as excursions that require parent consent as well as to notify our families about events such as incursions. We have already commenced using NForma Forms. When a new form goes out you will get an email to the address you have given the school.
TO Do - Parents are to check NForma Forms and either give consent or deny Consent.
This is essential for participation in excursions. Current forms that require consent are:
- Junior Gymnastics Clinics for Juniors only
- GIRLS ONLY - AFTERSCHOOL AFL for Girls only
- Multiculture Celebration for all students
- Grade 6 Fun Day for Year 6 students only
If you have not already downloaded the NForma Parent Portal please do this for access to school communication. When you have downloaded the app for the first time you will get a link that will use the gmail address you shared with the school. You will get a temporary password and will need to create one for future use.
