Asthma, Allergy and Medication Forms 

As the year draws to a close, I just wanted to remind you of a few important things for the upcoming school year.

If your child will need medication at school next year, please complete and sign a Medication Authority Form. For children with allergies or asthma, please provide the appropriate plan signed by your doctor.

Please bring the signed forms/plans and any labelled medication to the office on the first day of school next year.

If your child currently has medication at school, kindly pick it up from the office on the last day of school this year.

I've attached copies of the relevant forms for your reference.
