Student Medical Profiles

At Monivae College, the safety and wellbeing of all children is paramount in all aspects of school life. It is therefore very important to us that we are fully informed of any current medical conditions or concerns such as (but not limited to), asthma or anaphylaxis.

For staff to be prepared for an emergency we need to have an updated medical profile for your child, including all relevant and current medical information.

Please do this through the Parent Access Module prior to your child starting school in 2024.

We also require medical management plans for conditions such as asthma and anaphylaxis. These remain valid for 12 months only and are vital in providing us with strategies to best care for your son / daughter.

Please take the opportunity during the holidays to have these updated by your medical practitioner. Once completed please send in the management plan with your child at the beginning of the year.

To assist us with strict health and safety regulations, please ensure your child always carries an in-date Ventolin / salbutamol puffer and spacer with them.
