New Medication Authority Form

Please note that all new or amended student medications must now be accompanied by the updated Medication Authority Form. Families are asked to collect a copy of this form from the school office prior to visiting their doctor, to ensure it is completed and signed appropriately.

In addition, for any current medications that may be expiring or if you have not yet completed an Individual Management Plan for either Anaphylaxis or Asthma, please see Miss Jill at the office for assistance.
