First Aid - Medical Forms

Student Action Plans, Health Support Plans and Medication

As we begin the school year, we ask that families provide the school with current medical action plans and any required medication for their child. This includes (but is not limited to) plans for asthma, allergies/anaphylaxis, diabetes, epilepsy, or any other medical condition that requires monitoring or administration while at school. Students with complex medical needs must complete a Student Health Support Plan to provide for their specific health care needs.

If you are new to our school community, please forward your child’s current action plan, health support plan (if required),  medication and medication authority form as soon as possible so we can ensure your child’s wellbeing and safety while in our care.

  • Please ensure action plans are signed by a medical practitioner and are in date
  • Medication must be clearly labelled in original packaging with your child’s name and dosage instructions
  • Medication Authority Form must be completed by a parent/carer
  • Medication must be delivered to the school office by a parent/carer

If your child’s medical needs change at any time throughout the year, please notify us so we can update our records. Thank you for your support to help us maintain a safe environment for all students. If you have any questions, please contact the school office or e-mail brighton.sc@education.vic.gov.au .

School Uniform

Please support our drive to wear the uniform with pride in 2026!

  • Ensure that the correct and full school uniform is worn at school at all times, and it is in a neat and tidy condition.
  • The academic uniform is to be worn at all times other than when a student has PE or PASE on a given day.
  • The uniform is trans-seasonal which means the traditional summer or winter items can be worn throughout the year.
  • Sport and academic uniform CAN NOT be mixed.
  • The soft-shell jacket can be worn with either sports or academic uniform.
  • Any student who requires an exemption must have provided the school with supporting documentation from a medical professional (for example, a doctor’s letter if an item causes irritation of a skin condition). You do not need to re-send this if it has previously been supplied to the school.
  • BSC will work with students and families of anyone not in the correct uniform to ensure it is worn correctly and with pride.
